Placement Specialist vs. Geriatric Care Manager: What's the Difference

Placement specialist: helps you move in, once

A placement specialist’s job is narrow and time-boxed: understand your budget and care needs, tour or recommend a short list of communities, and help you get moved in. Most are paid a commission by the community you choose — typically a portion of the first month’s rent — not a fee from your family. Once the move happens, the relationship usually ends.

Geriatric care manager: helps you plan and oversee care, ongoing

A geriatric (aging life) care manager is typically a nurse or social worker, usually credentialed through the Aging Life Care Association, who does a clinical assessment, builds a care plan, and can stay involved indefinitely — coordinating doctors, monitoring a facility after move-in, or stepping in during a hospital discharge. Care managers are almost always paid directly by the family, hourly or by retainer, specifically so their recommendations aren’t shaped by which facility pays a commission.

Which one you actually need

• Just need to find and move into a facility fast — a placement specialist, understanding they’re paid by the community (see our guide to how that compensation actually works).
• Need someone managing care over time, not just a move — a care manager, especially for complex medical situations, out-of-town family, or an aging-in-place plan.
• Want an unbiased opinion on which facility, specifically — a fee-for-service care manager, precisely because they aren’t paid by any facility.

Some professionals do both, for a fee either way — always ask directly which model applies to your engagement before you start.
