What In-Home Care Actually Costs in California (2026)
In-home care pricing varies more by agency policy than most families expect — the hourly rate is only part of it. Here’s what actually drives the bill.
Hourly, live-in, and 24-hour rates work differently
- Hourly care is billed per hour, usually with a minimum shift (commonly 3–4 hours) — an agency won’t send someone for a single hour.
- 24-hour (shift) care uses two or three caregivers rotating shifts so someone is always awake — it costs meaningfully more than live-in because you’re paying multiple full shifts, not one flat day rate.
- Live-in care has one or two caregivers who sleep in the home and are paid a daily rate rather than hourly — it’s usually the cheapest per-hour option for round-the-clock needs, but California law requires the caregiver get an uninterrupted 8-hour sleep period and adequate meal breaks; frequent nighttime waking can push a case into 24-hour shift care instead.
What changes the rate
- Specialized dementia or Parkinson’s experience often carries a premium.
- Overnight, weekend, and holiday shifts are frequently billed at a higher rate.
- Some agencies charge a one-time assessment or care-plan setup fee.
- Rates in the Sacramento metro area (which Placer County is part of) tend to run somewhat below the statewide urban average, but still vary agency to agency — always get a written quote, not a phone estimate.
Ways to lower the cost without cutting corners
